WOCN OASIS Guidance Document Revised
The WOCN OASIS Guidance Document, originally developed in 2001, was updated and taken through the rigorous content validation process in July 2006 in order to reflect new evidence available in the last five years. We believe most clinicians will find the document easier to use, and they can be confident it has been updated to reflect the latest evidence available. The most notable change was the deletion of references to the term "healing ridge." This was done due to the lack of scientific evidence supporting the finding of the healing ridge in the assessment of surgical wounds closed by primary intention. Revisions include:
- All references to healing ridge have been removed
- The definition of avascular has been expanded to include "eschar and/or slough"
- The definition of pressure ulcer has been expanded to include the statement "shear and friction may be contributing factors"
- Text within the definitions has been reformatted to ease reading
